Demystifying the Smear Test: What to Expect and Why It's Changing

Let’s be completely honest: nobody looks forward to a cervical screening invitation landing on their doormat. It’s an appointment that often sparks anxiety, embarrassment, and procrastination. In fact, nearly one in three eligible people in the UK skip their test entirely.


In our latest episode, Dr. Geoff Hamp and Dr. Sam McGinley are joined by special guest Dr Helen Speakman and together they strip away the medical jargon and awkwardness surrounding cervical screenings. They break down exactly what happens during the appointment, explain how the science has quietly revolutionised over the last few years, and highlight why this simple test is actually a cancer prevention tool, not a cancer detector.



It’s an HPV Test Now


If you haven't had a smear test in a while, the way the lab checks your sample has changed. The NHS now uses HPV primary screening.

nstead of looking at your cells first, the laboratory checks your sample for High-Risk Human Papillomavirus (HPV)—a incredibly common virus that is responsible for 99.8% of cervical cancers.


Here is why that’s a game-changer for your screening schedule:


  • If you are HPV negative: Your risk of developing cell changes over the next few years is incredibly low. Because the science shows your protection lasts longer, the NHS extended the screening interval so you only need to go every 5 years (between the ages of 25 and 64).


  • If you are HPV positive: Don't panic. It does not mean you have cancer. It simply means the lab will take a closer look at your cells (cytology) to check for any abnormalities that might need a quick, simple treatment before they ever have the chance to turn into anything serious.


The 5-Minute Reality: What Actually Happens?


A lot of the fear surrounding the appointment comes from the unknown. Geoff, Sam and Helen walk through the procedural steps to show how quick and straightforward it really is:


1.The Pre-Check: 2 minutes.


The nurse or doctor will confirm your details, explain the test, and give you a private space to undress from the waist down.


Tip: Wearing a loose skirt means you only need to remove your underwear.


2.The Setup: 1 minute.


You'll lie on an examination couch, bend your knees, and bring your feet together. The nurse will use a speculum (a small, smooth plastic tool) lubricated with warm gel to gently open the vaginal walls so they can see your cervix.


3.The Sample: 30 seconds.


Using a tiny, soft plastic brush, the nurse will gently swirl the surface of your cervix to collect a few surface cells. It might feel like a strange scratch or mild cramp, but it shouldn't be painful.


4.The Clean Up: 1 minute.


The speculum is removed, and you are given privacy to get dressed. Your sample is popped into a small vial of liquid and sent directly to the lab. Results usually arrive by post within two to six weeks.


Comfort Tips from the Surgery


If you’re feeling nervous, Helan, Geoff and Sam share a few clinical tips to make your next screening much more comfortable:


  • Speak Up: Tell the nurse if you are nervous, have had a painful experience before, or suffer from conditions like vaginismus. They can use a smaller speculum or change your positioning.


  • The Breathing Trick: Pressing your lower back flat against the couch and coughing gently during insertion naturally helps relax your pelvic floor muscles.


  • Bring a Friend: You are completely within your rights to bring a chaperone, friend, or partner into the room with you for moral support.
